We now offer REBOOT Gift Cards available for use at our Functional Health Care Site + online.

*Gift Cards are non-refundable + not redeemable for cash. They must be presented at the time of intended use to be redeemed for health + wellness services. Gift Cards may not be used for medical appointments, co-pays, deductibles, co-insurance, account balances, or laboratory fees.
