Welcome to REBOOT and Functional Foundations. Below you will find our terms + conditions for our Functional Foundations tracks. By enrolling in a track or course with us, you agree to be bound by the following terms + conditions.

ENROLLMENT

Full payment is due upon enrollment and prior to any scheduling with our providers. Financing options are available upon purchase via Stripe. All payments are final once labs have been drawn.

You will be scheduled with providers best suited to meet your needs. Any requests to change providers will be granted only at the discretion of your provider.

All courses expire 120 days following your initial visit. All visits must be completed within this timeframe.

PATIENT PORTAL

You will have access to all Care Plans and visit summaries via a patient portal account in our electronic health record, Athena. Our scheduling team will assist you in creating this account upon enrollment.

Best practice for use of your patient portal include:

• Before sending a question on the patient portal, please carefully review your most recent care plan and all associated care plan documents to ensure these do not already answer the question.

• The patient portal is meant for brief questions and answers around clarifications or updates to the patient’s existing care plan / treatment recommendations, testing results, and overall healing progress.

• Patient portal messages are not to take the place of office visits.

• The patient portal is not meant to discuss:

  • New clinical concerns or problems not previously discussed in an office visit

  • Questions about the health of people other than yourself

  • Extensive or complex updates / questions on care that require an office visit to thoroughly and responsibly discuss and manage.

  • If such concerns arise, please contact your health coach for next steps.

• Allow 48 business hours for your Care Team to acknowledge receipt of any portal message and 72 hours for your message to be fully addressed.

CONFIDENTIALITY

Many Functional Foundations visits take place remotely via our telehealth platform in Athena. If this is the case for your visit, please ensure you are in a quiet, private space during your telehealth visit to protect your privacy. REBOOT is not responsible for any unsecure locations or internet connections you may have during your telehealth visits.

It is understood that you will be working with a team of providers during your course. These providers will have access to your records from the course and will discuss your progress + protocols together as a team to provide you with the best team approach to your health.

SUPPLEMENTS

You care team may recommend supplements as part of your Care Plan. They will provide you with recommendations to purchase your supplements via our online dispensary partner, Fullscript, where you will receive a discount on products purchased. You do not have to purchase your products through our Fullscript partner. Please be aware that if you choose to purchase recommended products through this partner, your provider will receive a small commission on your order.

LIMITS

Functional Foundations is a personalized educational course delivered by licensed medical providers. Your Functional Foundations care team will not serve as your primary care provider. Should you choose to become a member of our Integrative Primary Care team, we will be happy to enroll you for ongoing care. Functional Foundations does not provider acute or emergency medical care and does not provider after hours communication. In the event of an emergency, please call 911 or present to your nearest emergency department.
